Insurance & Financial Policy
As part of our COVID-19 safety precautions, we have stopped collecting physical insurance cards. Please make sure to fill out your paperwork entirely, along with uploading your insurance card by clicking the button at the bottom of this page to upload images of your cards. Having correct insurance information is the only way our billing team is able to provide an estimate for your procedure.

To accomplish this goal, we work hand in hand with both medical and dental policies to maximize your insurance reimbursement for covered procedures. Don’t see your insurance on that list?
If we do not participate with your insurance plan, we will still file a claim to your insurance for you. However, you remain financially responsible for all charges incurred. We are participating providers with Medicare. Medicare does not cover most dental services, however, there are certain oral conditions that Medicare deems medically necessary and will cover. Medicare patients are responsible for:

- Non-covered services

Workers Compensation
Written authorization must be obtained from your employer before treating a Workers Compensation injury. Claims for work related injuries are sent to the insurance company or your employer. If not covered by insurance, you agree to be responsible for the balance due for the services rendered.
Financial Information
For your convenience, we accept Visa, MasterCard, American Express, Discover and CareCredit. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.
